(To be used, in duplicate, in all cases of discharge on account of disability)
William Sours a Corporel (sic) of Captain (blank) Company, (E) of the 14th Regiment of Iowa Vol. Infty was enlisted by J. H. Horine of the 14th Regiment of Iowa Vol Infty at Vandalia Iowa on the 28th day of September 1861, to serve 3 years; he was born in Summit Co. in the State of Ohio is 24 years of age, 5 feet 9 3/4 inches high, Fair complexion, blue eyes, Dark hair, and by occupation when enlisted a Carpenter. During the last six months said soldier has been unfit for duty 160 days. (Here consult directions on Form 12, p. 269, Medical Dept. Gen. Reg.) Owing to sickness contracted since his enlistment and while he was a prisoner of war in Macon Georgia where he was kept During last Summer and up to about the 1st day of Oct 1862 Since which time he has not been able for duty at any time.
